Passenger Transport Subsidy Scheme Assistance

Eligibility Overview
- Australian citizen or permanent resident
- Residing in Western Australia
- Severe and permanent disabilities preventing use of conventional public transport
- Mobility disability, vision disability, or cognitive/intellectual disability
Application Checklist
- 1 Complete Part A of the application form (applicant details)
- 2 Book an appointment with a medical practitioner
- 3 Attach identity documents (as required by the form)
- 4 Attach a certified colour photograph
- 5 Medical practitioner completes Part B
- 6 Submit application to Department of Transport by email or post

Medical Practitioner Section
